- Birth-registration certificate
- Copy of the father's and/or mother's citizenship
- School certificate (showing date of birth)
- Recommendation from your ward office

- 1 Get a recommendation from your ward office.
- 2 Submit the filled form and documents at the DAO.
- 3 Verify in person before the officer / CDO.
- 4 Collect your citizenship certificate.
